Hayat Disease Surveillance is a digital surveillance tool that health workers and designated community volunteers at AKHS, P clinics and labs use on tablets.

Its focuses on tracing and collecting data and disease outcome information on patients suspected or confirmed to have COVID-19, whether they are being cared for at home, referred for advanced care, or have received lab results (PCR and immunoassays). The app also traces close contacts of COVID-19 patients.

AKDN dHRC developed the app in collaboration with AKHS, P to support population-level disease surveillance and reporting on COVID-19 in remote areas of Gilgit-Baltistan and Chitral. The app's database and reporting portal track COVID-19 cases, their progress, and isolation efforts, while also identifying hotspots for policymakers to design effective response strategies, guide prevention efforts, and conduct early alarm activities like vaccination and sero-surveillance.

The Hayat Disease Surveillance has registered 4,776 individuals to date and is a component of the Hayat app, which community health workers in select regions of Gilgit-Baltistan and Chitral currently use.
